Project Abstract

The Diabetes Lifestyle Management Program will utilize two Community Health Workers (Lifestyle Coaches) to provide information and activities to prevent the onset of diabetes in the African American community.

The project will reduce barriers to care, help patients’ access service and support participants healthy lifestyle behaviors/goals related to diet, exercise, and weight management.

Central Ohio Diabetes Association (CODA) will partner with Columbus Neighborhood Health Centers, Inc. (CNHC).

The goal of the Stepping Out Diabetes Prevention Program “Saliendo Afuera:  Programa de Prevencion en la Populacion Minoria” is to prevent, reduce or delay the incidence of diabetes among African American, Hispanic and other minority residents in the city of Youngstown.

The program will provide resources, diabetes prevention education, physical activities, weight loss programs and structured motivation sessions for 300 program participants.

Project Abstract

The “After the Baby is Born” project will provide care coordination during the first six months of life with a goal of improving the health of babies of African American women in Lucas County.

The project will contribute to the prevention of infant death by providing social support and health education on infant care, safe sleeping, smoke detectors, tobacco free environments, shaken baby, and SIDS to 100 women and their significant other.

The four organizations providing care coordinators are Neighborhood Health Association, St. Vincent Mercy Medical Center, The Toledo Hospital and the Toledo-Lucas County Health Department.

Project Abstract

The Health and Activities Promotion Initiative (HAPI) is a linguistically appropriate and culturally competent Asian American Diabetes prevention, education, outreach and support program.

It is estimated that nearly 3,000 Asian American/Pacific Islander (AAPI’s) in Northeast Ohio live with diabetes.   The highest rates are among Asian Indian, Filipino, Korean and Southeast Asians.

HAPI will use a multi-pronged approach to eliminate health disparities in the area of type 2 diabetes.

Project Abstract

Infant mortality rates in Hamilton County and Cincinnati have been consistently higher than in other urban communities in Ohio.

Maternal depression is a major associated factor occurring prior to infant loss (25% of Cincinnati case reviews documented depression compared to national prevalence rates of 13%), recognizing that depressed mothers are less able to care for both themselves and their families.  Local data suggests that maternal depression occurs at very high rates and remains untreated (45% of mothers in a home visiting program were identified as having perinatal depression, and 80% of those mothers could not access mental health treatment).

The grant will train primary care providers at community health centers across Hamilton County to screen for, diagnose and treat perinatal depression.

Project Abstract

The Lorain County Urban League (LCUL) will implement a diabetes prevention project that targets African American and Hispanic families in Lorain County.  The Save Our Families project will have sites in Elyria’s Southside, Central and South Lorain and the Southeast Quadrant of Oberlin. 

LCUL will recruit and train four community health workers and 10 Service Learning students from the nursing program at Lorain County Community College to teach families how to make healthy food choices, exercise and avoid risk factors that lead to the onset of diabetes.
